The following information is provided to you in order to make your visit to our office as convenient for you as possible. Please call your insurance company to check your benefits.
New Patient Checklist
- Please return the enclosed Registration and HIPAA forms either by mail or in person when you come for your initial visit. (Please click HERE to download and print the required forms).
- On your testing visit, please allow a maximum of 3 hours for the appointment.
- If for any reason, you cannot keep your appointment, please notify us as soon as possible so we may free up the space for another patient in need of care.
- A charge of $25 will be applied if you do not give 24 hours notice of a cancellation.
- A complete allergy survey may require at least one additional trip to our office. This will vary depending on the patient.
- Copies of any lab work, x-ray reports, previous allergy studies, or other pertinent medical studies may be important and should either be sent beforehand or brought along.
Please stop taking the following medications for the amount of time specified:
CANNOT HAVE THE FOLLOWING:
- Claritin/Loratidine – 5 days
- Xyzal /Levocetirizine – 5 days
- Allegra/fexofenadine – 5 days
- Zyrtec/Cetirizine – 5 days
- Clarinex/Desloratidine – 5 days
- Amitriptylline – 5 days
- Any over the counter cold and cough, sinus medication such as Tylenol Cold and Sinus, Dimetapp
- Certain antidepressants (Elavil, Sinequan (Doxepin), Imipramine) – 5 days
- Other Antihistamines:Benadryl (dyphenhydramine), Atarax (hydroxyzine), Phenegran (Promethazine), Dallergy, Bromfed, Chlor-Trimeton, & Actifed, etc – 3 days
- Over-the-counter sleeping medication – 3 days
- Other drugs to avoid for 48 hours prior to appointment: Zantac (Ranitidine), Pepcid (famotidine) and Tagament (Cimetidine HCI)
- All other medications including other antidepressants not listed above should be continued as prescribed. If you have any questions regarding a specific medication, please consult your pharmacy or prescribing provider.

- Dulera, Advair, Q-Var, Singulair, Symbicort, & all nasal sprays
Insurance
We participate with most major carriers. Please call your insurance to verify if AASC is in-network with your specific plan. You must call your carrier for your specific allergy benefits, deductible amount, and referral/prior-authorization requirements.
- ALL children under the age of 18 must be accompanied by a parent or guardian on his/her initial visit.
- Make sure we have your current insurance information and a copy of the current card.
- Please be prepared to pay your copayment in full at the time of service.
- Medicaid patients must have their current Managed Care Card (if applicable).
- Be sure your referral is up to date. Contact your referring physician to ensure that he/she has sent us a referral that includes the date of your visit.
- AASC will work with you to resolve your balance, and we have a patient representative who can assist with developing a payment plan. We also partner with Care Credit for larger sums. Below is a link to Care Credit’s site for more detailed information.
Please Remember
Your insurance is a contract between you and your insurance carrier. You are responsible for any coinsurance, copay, deductible, or cost share unpaid by your plan. It is also your responsibility to ensure valid referrals are in place at the time of service.
